Deposit: Secure an appointment a $1,000 non-refundable deposit is required to hold the date. Should the appointment be canceled, the deposit is non-refundable. The deposit is subtracted from the evaluation fee.

Fees:  The fee for the comprehensive evaluation is $4,250.00 (Effective March 22, 2024).

Payment: Master Card, Visa, American Express, Discover, Zelle, Paypal. Financing option is available. Personal checks are not accepted. Balance on the account is due on day of testing .

Fees include: The fee for a comprehensive evaluation includes approximately up to 1-2 hours of parent/client consultation (email, phone, face to face), 6-8 hours or more  of assessment, 4-6 hours or more of data interpretation/analysis including additional 1 hour of licensed clinical psychologist collaborative supervisory review, 4-12 hours report preparation and writing. Without exception, findings will be shared only after final payment has been received. Additionally a follow-up appointment is available for scheduling that includes up to 1 hour of face-to-face, email and/or telephone discussion of findings and recommendations. Should a second feedback follow up meeting be requested an hourly fee will be applied.

Insurance: At this time School Bridge, LLC does not accept assignment of insurance benefits. The client is responsible for payment. Due to the ever changing health insurance laws and regulations, we will not guarantee that services will be covered by your insurance policy. However clients are encouraged to submit receipt for services to their insurance company for possibility of reimbursement. Once the evaluation is completed, and paid in full, a receipt with diagnosis and CPT codes required by insurance companies will be provided.
